Help with a Federal Agency
If you can't get an answer from a federal agency in a timely fashion, or if you feel you have been treated unfairly, our office may be able to help resolve a problem or get you the information you need. While we cannot guarantee you a favorable outcome, we will do our best to help you receive a fair and timely response to your problem.
Residents of the 27th Congressional District of Florida can contact us for assistance in dealing with Federal agencies.
Please include all pertinent information and claim numbers in your correspondence—such as:
- Your Social Security number for a case involving Social Security;
- VA claim number for a case with Department of Veterans Affairs;
- Taxpayer identification number (Social Security number, if individual) for an Internal Revenue Service problem, etc.;
- Your address, home phone number, and daytime phone number (if different than home) so that we can obtain any additional information from you that might be necessary;
- Copies of any related documents or correspondence that you may have from the agency involved;
- Front and back copies of your Florida-issued ID must be submitted with the Privacy Release Form.
For U.S. Citizenship and Immigration Services (USCIS) cases, please read the following.

- Receipt/Tracking Numberis required (starts with three letters followed by 9-10 digits, e.g., IOE1111111111)
- Receipt Notice, Notice of Action, Form I-797, must be uploaded/attached while completing the electronic form.
- If the applicant or beneficiary is not a United States Citizen, the Alien Number is required.
- The Privacy Release Form must be in English
- The form should be signed by the person named on the receipt notice.

DHS ALERT: DHS has temporarily paused the issuance of Advanced Travel Authorizations (ATA) for new CHNV beneficiaries while it undertakes a review of the supporter application process. USCIS continues to accept Form I-134A, however, no requests will be confirmed until the review of the supporter application process is complete. Beneficiaries who have already received an ATA should check their myUSCIS account for updates before making travel arrangements and prior to travel. Individuals with a valid ATA are permitted to travel. ATA processing will restart as soon as possible once this thorough review concludes.
